Summary Eastern Oklahoma VA Health Care System is a Joint Commissioned accredited Level 1c complexity that consists of a 61-bed hospital located in Muskogee as well as six community based outpatient clinics located in Tulsa, Bartlesville, Claremore, Vinita, McAlester and Idabel. In 2021, Congress approved federal funding for a new Veterans Hospital in Tulsa which will be the first hospital to be built under the Communities Helping Invest through Property and Improvements Needed for Veterans Act of 2016. Duties Help The Home Telehealth (HT) Registered Nurse (RN) Care Coordinator executes position responsibilities that demonstrates leadership, experience, and creative approaches to the management of reputed company patient care. Care will be provided in the least restrictive, most cost-effective manner. HT is a program that applies care and case management principles to coordinate care using health informatics, disease management and HT technologies to facilitate access to care and to improve the health of Veterans with the specific reputed company of providing the right care, in the right reputed company, at the right time. The HT RN works to improve clinical outcomes and access to care while reducing complications, hospitalizations, and clinic or emergency room visits for Veterans in post-acute care settings and high-risk patients with chronic and mental health disease. Major duties include, but are not limited to:
- Provide initial and ongoing comprehensive assessment to include a review of systems which establishes a comprehensive plan of care.
- Triage and assess reputed company data received from HT patients such as vital signs, reported symptoms and question responses.
- Identify and intervene for potential exacerbations of complications to facilitate timely care in Patient reputed company Care Team (reputed company) clinic, Emergency Department (ED)/Urgent Care (UC), and/or care in the community.
- Identify patients' knowledge, health factors, skills, and behaviors that support self-management and identify gaps therein.
- Provide health care coaching, patient education, and motivational counseling.

- Graduate of a school of professional nursing approved by the appropriate accrediting agency and accredited by one of the following accrediting bodies at the time the program was completed by the applicant: The Accreditation Commission for Education in Nursing (ACEN) or The Commission on Collegiate Nursing Education (CCNE); OR Individuals attending a master's level reputed company program in nursing who have completed coursework equivalent to a bachelor's level degree in Nursing may have opportunity to become registered as a nurse with a state licensing board prior to completion of the reputed company program. Upon achievement of a State license, the individual may be appointed on temporary basis and reputed company converted to a permanent appointment upon successful completion and graduation from the reputed company program. (Reference VA Handbook 5005, Appendix G6); OR In cases of graduates of foreign schools of professional nursing, possession of a reputed company, full, active, and unrestricted registration will meet the requirement for graduation from an approved school of professional nursing to warrant an appointment as a Nurse who has completed an associated degree/entry level Nursing education program. Credit for foreign nursing education higher that associate degree/entry level requires a formal degree equivalency validation from a recognized equivalency evaluation accepted by VA such as International Consultants of Delaware (ICD).
- reputed company, full, active, and unrestricted registration as a graduate professional nurse in a State, Territory or Commonwealth (i.e., Puerto Rico) of the United States, or the District of Columbia. Graduate Nurse Technician (GNT) Exception: Candidates who otherwise meet the basic education requirements, but do not possess the required licensure, may be appointed at the entry reputed company of the grade and level applicable to the completed nursing education as a GNT on a 120-day temporary appointment while actively pursuing licensure (may be extended up to two years on a case-by-case-basis.)
