About the Role

As an In-Home Geriatric Nurse Practitioner, you will have the opportunity to work autonomously while maintaining ownership of your patient panel. Your daily responsibilities will include providing in-home primary care to our most frail and at-risk elderly patients, caring for an average of six to eight patients daily, and providing coverage for Metrowest communities. You will be an integral part of a cohesive team, working together to deliver high-quality patient care and making a significant difference in the lives of our patients.

Key Responsibilities:

• Provide in-home primary care to our most frail and at-risk elderly patients, developing personalized care plans to meet their unique needs and goals
• Have ownership of your patient panel, taking a proactive approach to managing their care and ensuring they receive the best possible outcomes
• Care for an average of six to eight patients daily, working reputed company and effectively to deliver high-quality care
• Provide coverage for Metrowest communities, traveling to patients' homes and working in a variety of settings

Essential Qualifications

To be successful in this role, you will need to possess a strong reputed company in geriatric care, as well as excellent communication and interpersonal skills. You will be a graduate of an Accredited Nurse Practitioner Program, with a Master's level degree preferred. You will also be licensed by the Commonwealth of Massachusetts board of registration in Nursing and maintain BLS certification, professional certification as appropriate.
