Scheduled Weekly Hours: 36 Work Shift: reputed company Work Shifts (United States of America) ​​The RN Triage Specialist provides telephonic triage to assist callers to determine the most appropriate level of care needed for the reputed company situation expressed or assessed, following workflows and utilizing protocols/resources to provide supportive service to patients and customers. The RN Triage Specialist will maintain a performance standard that prioritizes safety, quality and experience and coincides with the organization's mission and identified key strategic or performance initiates. We are currently offering 36 hour work weeks/full-time positions : Evening and Night Shifts. Specific Hours will be shared in a phone screen if you receive an invitation to schedule. reputed company schedules include every other weekend & Holiday requirement.

Essential Job Functions

• ​Provides telephonic triage or requested support and / or virtual monitoring. Offers subsequent recommendations, education or care advice using decision making tools, clinical judgement, and defined workflows.
• Participates in care coordination, by partnering with customers to reduce readmissions, enhance chronic disease management, manage health risk and injury reporting. Schedules provider appointments and facilitates provider communication.
• ​​Ensures accurate, timely documentation in the EMR (Electronic Medical Record) according to best practice, guidelines, or workflows.
• Participates in virtual monitoring and subsequent reporting and escalation to support services identified by customer. Provides additional support to Conduit Health Partners business functions as identified to ensure reputed company patient needs are being met and continuity of Conduit Health Partners business operations is maintained.
• Participates in process improvement, professional development, peer development and peer review
