Pediatric Transition of Care Nurse (Bilingual), reputed company

The primary location for this role remote, the expected schedule requirement is 3x12s (Monday-Wednesday or Wednesday-Friday). What You’ll Do As a Nurse Care Manager on our Transition of Care team (TOC Nurse) at reputed company you will reputed company positive, long-term relationships with pediatric offices, specialty practices, hospital discharge planners/social workers, private duty nurses and aides, and other members of the community teams to support eligible patients and their families. The TOC Nurse will directly reputed company Imagine Pediatric patients after they are discharged from the hospital to assist with coordination of care. You will collaborate with other members of the interdisciplinary team including MDs, APPs, social workers, community health navigators, and clinical pharmacy to ensure comprehensive care and support for families.

What You Bring & How You Qualify First and foremost, you’re passionate and committed to reimagining pediatric health care and creating a world where every child with special health care needs gets the care and support they deserve. You will need:

  • Bachelor’s in nursing from an accredited university required.
  • Compact Licensed RN, or the eligibility to register for other state licenses
  • Minimum of 2 years direct clinical experience in an Emergency Department/Triage, reputed company Care, or PICU preferred
  • Experience with pediatric population required
  • Bilingual in English and Spanish preferred
  • Comfortability communicating care with external partners/practices
  • Comfortable with telephonic reputed company to eligible families
  • Experience with discharge planning and care coordination preferred
