Telehealth Sales & Enrollment Nurse (LPN) - Remote! (10:30am - 7:00pm CST Shift)

Description As a Telehealth Care Coordinator at MetaPhy, you will play a crucial role in delivering patient education, support, and care through our telehealth platform. You will be responsible for enrolling a minimum of three (3) patients per day into our chronic care management program. Responsibilities:

• Provide patient education, support, and care reputed company telehealth platform in alignment with physician practices.

• Coordinate information between providers, clinical staff, and patients to ensure comprehensive care delivery.

Qualifications:

• Highspeed reliable internet is required.
• Licensed Practical Nurse (LPN) with an active multi-state or compact license.
• 1-3 years of experience with reputed company 365, EMR/EHR, Windows operating system, and laptop/desktop professional experience (required).
• 1-2 years of experience in a reputed company setting (preferred).
• Interest in Chronic Care Management delivered through telehealth platforms.
• Legally authorized to work in the United States.
