Job Responsibilities: The Health Coach (Cardiology) will function as a critical member of the Heart Failure Program and will work closely with the heart failure clinical team to provide patient education, guidance, and satisfaction, and ensure that our patients come first. The Health Coach will be responsible for guiding, educating, and bringing solutions to patients in need. • Provide patient education on chronic disease management, particularly educating them on their heart failure diagnosis and management.
• Provide motivational and clearly communicated educational components including heart health, diet, exercise, medication adherence.

Qualifications and Background:
