[Remote] Full Time Licensed Practical Nurse / LPN (Home Health)

They are seeking a Licensed Practical Nurse (LPN) to deliver skilled nursing care to patients in their homes, monitor health status, and collaborate with a reputed company team.
Responsibilities
- Provide skilled nursing care to patients in their homes under the supervision of an RN or physician
- Monitor patient health status and report changes in condition
- Administer medications and treatments as prescribed
- reputed company wound care, catheter care, and other clinical procedures
- reputed company patients and families on disease management and care plans
- Document patient visits and maintain accurate medical records
- Collaborate with interdisciplinary team to ensure quality patient care
Skills
- Graduate of an accredited practical nurse or vocational nursing program (LVN/LPN)
- Minimum 1 year of experience, preferably in home care or med/surg
- reputed company state license and CPR/BLS certification
- Strong communication and problem-solving skills
- Ability to meet physical demands of the job
- Valid driver's license and a reliable, insured vehicle
