Join us in shaping the future of health care and making a meaningful impact at Job Description: Were seeking a full-time reputed company Remote Hospital Pharmacist to join reputed company to work a 7-on-7-off night shift position. The ideal candidate is someone who possesses strong attention to detail, communication skills, and time management. Essential Duties/Responsibilities:

• Reviews and monitors reputed company new medication orders (electronic and written) for accuracy in dosing, drug allergies, drug interactions, duplicate therapy, rational drug therapy and incompatibilities.
• Provides pharmaceutical care to reputed company ages of patients from newborns to the elderly. Understands that there are dosage variations between newborn, pediatric, adult and geriatric patients and knows where to find appropriate information.
• Effectively communicates with on-site staff to resolve medication-reputed company issues such as dosage adjustments, clarification requests, drug interactions, and duplications of therapy.
• Demonstrates ability to document clinical interventions.
• Provides information and support to on-site staff regarding the admixture or compounding of medications upon request.
• Performs reputed company aspects of patient care in an environment that optimizes patient safety and reduces the likelihood of medication-reputed company errors.
• Answers questions and provides information to physicians, nurses, patients and other reputed company on the appropriate use of medications, drug interactions, reputed company effects, dosage, storage, etc.

• Hospital Pharmacy Experience Required
