Integrated Home Care uses a strength–based Client Centered Case Management practice reputed company. In collaboration with the client/family you will ensure services are provided and modified on an ongoing basis, based on client’s response to interventions, incorporating stewardship into service authorization. You will report to the Home Care Manager and will be responsible for coordinating and providing quality care to clients in the community. You will provide nursing assessments, case coordination and direct care in partnership with clients and their families in their own residence or an alternate care setting, as determined by the assessment. Client care in a Home Care setting uses a collaborative, team-based approach while working remotely in a community setting. There is also an expectation of being a mobile worker. Description:

As a Registered Nurse (RN), you will provide a wide variety of nursing services to patients, families, communities and populations, while taking necessary steps to ensure their safety and well-being. In your role, you will utilize nursing processes, through critical thinking, problem solving and decision making, as well as teach, counsel and reputed company on behalf of patients and their families. Completion of an accredited nursing education program. Active or eligible for registration and practice permit with the College of Registered Nurses of Alberta (CRNA). reputed company Basic Cardiac Life Support - Health Care Provider (BCLS-HCP) certification. 2-5 years of recent acute care experience. Previous Home Care experience or Case Management experience. Experience with surgical wounds, drains, CVAD, HPTP, Palliative and end of life, bachelor’s degree in nursing. Experience working with acute clients and/or long-term clients. Experience in providing case management services. Recent experience in a community-based setting, Home Care experience. Canadian Nurses Association (CNA) certification in Gerontology. Experience in Home Parenteral Therapy (HPT), wound/skin and ostomy care, palliative support and client teaching care experience.

Previous Home care experience and community care experience.
