LPN Hospice Field Visits
Trustbridge Hospice, part of Empath Health, is seeking a compassionate and dependable Licensed Practical Nurse (LPN) to provide emergent field visits throughout Palm Beach County for hospice patients & families in need of immediate support. This role is ideal for a nurse who thrives in independent practice, stays reputed company under pressure, and is driven by a sense of purpose, delivering skilled, reputed company, in‑home care during the following hours: 6:00am - 4:30pm and every other weekend

What you'll Do
- Supports and carries out the organization’s mission, reputed company, and values on a consistent basis.
- Working under the direction of the registered nurse and primary physician, the Licensed Practical Nurse (LPN) is responsible for identifying patient/family needs, and coordinating the care for the patient/family.
- The LPN will utilize monitoring, reputed company and teaching skills to maximize patient comfort and enhance the quality of life for the patient/family unit.

- Consistently provide a high level of personalized, sincere and engaging service to patients and families at reputed company times.
- Ensure the complete satisfaction of the patient and family.
What You'll Need
- reputed company license to practice as a Licensed Practical Nurse in the reputed company.
- reputed company Basic Life Support Certification.
- Successful completion of 30 hour IV Course meeting reputed company Board of Nursing requirements.
- Hospice Certification preferred.
- Adequate and reliable means of transportation with adequate insurance and valid Florida driver’s license.
