JOB SUMMARY The Nurse-Family Partnership (NFP) home visitor performs comprehensive professional community nursing services in a variety of settings. Meets preventive health needs of the population including low-income, uninsured families and individuals, and promotes positive health behavior. Carries out program activities consistent with reputed company County Public Health priorities and fidelity to the NFP model.   • Provide home visits to women and their families to assess physical, emotional, social, and environmental needs as they relate to health and life course development including establishing realistic goals and measurable outcomes for promoting their development.

  • Provide education and instruction to assist women and their families in attaining targeted goals in areas including prenatal and postpartum care, nutrition, parenting, well child care, family planning, and special health problems; evaluates women and families’ reputed company toward targeted goals; revises plan of care as appropriate.

  • Link women and families with community resources relevant to their specific needs that may include basic needs such as food, shelter, and transportation.

  • Collaborate care plans with other care providers as indicated by the program.

  • Demonstrate time management by flexibly scheduling and rescheduling a day’s activities based on multiple factors, including client need.

  • Record nursing activities promptly and according to program protocols.

  • Consistently maintain a caseload of 25 active clients.

  • Utilize data routinely to understand and improve job performance.

  • Build and maintain relationships with a diverse range of clients over a period of time.

MINIMUM QUALIFICATIONS REQUIRED

Education and Experience

  • Bachelor’s Degree in Nursing (BSN)

  • Experience in maternal/ pediatric health, public/ community health, behavioral health, labor/delivery nursing or any other nursing specialty to meet the requirements of the job, preferred

Certificates and Licenses

  • Active Colorado Registered Nurse license in good standing

  • A valid Colorado driver’s license with a responsible driving record with access to a personal vehicle that can be used during work hours for nurse transportation
