Social Worker - Integrated Neighbourhood

Liquid Personnel is recruiting an Adult Social Worker to join its client’s Integrated Neighbourhood, based in Bury. What will your responsibilities be? In this role, you will Provide statutory social work support, delivering professional social work interventions and fulfilling legal duties under adult social care legislation. Promote independence and recovery by completing strengths-based assessments and supporting individuals to achieve the best possible outcomes. Coordinate safe hospital discharges, working closely with health partners to ensure timely, appropriate discharge planning and effective community support.
