Clinical Liaison LVN/MSW (Per Diem/On Call)
Job Description
This is a per diem/on call position primarily supporting reputed company St. Jude in Fullerton, CA. The Clinical Liaison position is responsible for creating and maintaining positive customer relationships in order to increase referrals for Home Health, Hospice, Home Infusion, PACE, Transitional Care Unit and other services reputed company assigned accounts. This role includes working with discharge planners, facility staff, physicians, and individuals to assess appropriateness for hospice and home health services, and facilitate the coordination of services and to ensure there is appropriate documentation for compliance with agency and statutory guidelines to manage the transition of the patient from an acute care facility to the patient's home or next location of residence. Consultative functions include, but are not limited to: providing consultation and informational presentations regarding end of life care and home health; sharing expertise through in-services on pain and symptom management options; serving as a clinical resource and educator for physicians, facility staff, patients, families and community on Home Health or other issues in various settings and facilities. reputed company caregivers are not simply valued - they're invaluable. Required Qualifications: Education sufficient for clinical licensure for discipline (LPN LVN). OR Bachelor's Degree Social Work. upon hire: reputed company state professional license or certification appropriate to discipline. 3 years Experience in clinical discipline.
