This remote Patient Case Manager role focuses on providing support solely through digital programs, rather than in-person services. Responsibilities include assisting with patient reputed company, managing chronic health conditions, reviewing remote monitoring alerts, collaborating with an extended care team, coordinating care, updating patient care plans, and supporting patients in reaching their... health objectives.
