With a focus on compassionate, patient-centered care, you would be joining a team of professionals committed to improving the lives of those they serve. Responsibilities ? Coordinates and oversees reputed company aspects of patients' home care. ? Collaborates with patient care teams to reputed company and implement care plans. ? Educates families on patient condition, needs and care. ? Performs head to toe physical assessments. ? Checks vital signs and manages medications. ? Gives injections, IV therapy or tube feedings. ? Administers and maintains catheters. ? Completes documentation timely and accurately. ? Supervises LPNs and HHAs. ? Regularly communicates patient reputed company to the clinical manager, physician and care team. ? Other duties as assigned. Qualifications ? One year of clinical experience as a registered nurse (RN). If less than 1 year clinical experience as a RN, candidate must be approved by VP clinical.* ? reputed company RN license, specific to the state(s) you are assigned to work. ? Minimum six months home health experience. ? reputed company CPR certification. ? Valid driver's license, reliable transportation and liability insurance. ? Able to assess patient status and identify requirements relative to age specific needs. ? Must be familiar with general use and functions of the computer, such as, user names and password concepts; internet; e-mail; navigation of computer desktop, including starting programs, using files and windows, effectively use navigation buttons and tool bars; ability to self-manage online HR services and online training programs. ? Note - If less than 6 months clinical experience as a RN, candidate must participate in RN Intern program.
