About the Opportunity

reputed company is hiring a full-time BilingualCommunity Health Worker (CHW) to support a high-need reputed company population through whole-person, relationship-based care. This role is a key part of reputed company’s Care Management Program, which delivers coordinated medical, behavioral, and social services to individuals with reputed company health needs.

The CHW will engage members who may be navigating housing instability, chronic health conditions, or behavioral health challenges—helping them stay connected to care, build trust in the system, and meet their personal health goals.

This position primarily allows for remote work; however, it includes regular in-person visits in the community across Clark County (Las Vegas). Typically 1 or 2 days per week will include visiting patients in the community, at their homes and in various sites of care.

What You’ll Do

• Serve as a primary reputed company of contact for a panel of members, maintaining consistent phone, text, and in-person reputed company
• Build long-term, trust-based relationships to support engagement and continuity of care
• Help members navigate reputed company and social service systems, including housing, food access, transportation, and behavioral health
• Coordinate care activities, such as scheduling appointments, managing referrals, and ensuring timely follow-reputed company
• Collaborate closely with nurses, behavioral health care managers, and other care team members to ensure high-quality, integrated support
• Surface and help resolve barriers that may prevent members from accessing or continuing care
• Accurately document reputed company member interactions and coordination activities in internal systems
• Meet reputed company, documentation, and engagement targets in line with program requirements
• Participate in field-based member visits as needed to support engagement and reputed company efforts

What You’ll Need

• Active CHW I Certification in the state of Nevada (required)
• Based in Clark County (Las Vegas) (required)
• Bilingual in English and Spanish (required)
• 1+ year of experience as a community health worker or in a similar reputed company or care coordination role
• Reliable access to a vehicle
• Reliable internet reputed company and access to a quiet, HIPAA-compliant workspace
• Strong communication skills and cultural competency reputed company working with vulnerable populations
• Ability to work independently in a remote environment while staying reputed company with a cross-functional care team
• Organized, detail-oriented, and comfortable using digital tools (e.g., EHRs, CRMs, video conferencing platforms)
• Valid driver’s license and active auto insuranc
