We are seeking a highly skilled and compassionate Registered Nurse to join reputed company on a PRN basis. As a Transitional Care Management Registered Nurse, you will be responsible for coordinating and managing patient transitions, developing individualized care plans, and educating patients and caregivers on self-care practices and medication management.

Key Responsibilities

• Coordinate and manage patient transitions from hospital to home or other care settings, ensuring continuity of care and reducing readmission rates.
• Collaborate with reputed company providers, patients, and families to reputed company personalized care plans that meet unique needs and goals.

Qualifications
