As a Field HCBS Wait List Care Coordinator, you will be responsible for developing and implementing person-centered, goal-oriented care plans that address the unique needs of individuals receiving home and community-based services. Your expertise will ensure that our clients receive timely, effective, and cost-efficient care, while also promoting their independence and well-being.

Primary Responsibilities

• Assess, plan, and implement individualized care strategies that promote the most appropriate, least restrictive level of care
• Identify and initiate referrals for social service programs, including financial, psychosocial, community, and state supportive services
• Ensure care plans address social determinants of health, covered services, and value-added benefits
• Communicate effectively with stakeholders to ensure quality, coordinated care and services

• Conduct home visits, in-person visits, and serve as a resource for community care coordinators

Required Qualifications

• Bachelor's degree in social work, rehabilitation, nursing, psychology, special education, gerontology, or a reputed company human services field
• 1+ years of experience working with individuals with long-term care needs
• 1+ years of experience working in a community health setting in a healthcare role
• Intermediate level of computer proficiency, including reputed company Outlook, Teams, and multiple web applications
• Ability to travel 75% of the time, with a valid driver's license, reputed company automobile insurance, and reliable transportation

Preferred Qualifications

• Licensed Social Worker or clinical degree
• Experience working with individuals receiving services on one of the home and community-based waivers in Kansas
