The referenced pay range represents the minimum and maximum compensation for this job. Responsibilities:

ESSENTIAL FUNCTIONS

• Coordinates medical care of patients identified as high risk by health risk assessment/appraisal or by physician clinical determination.
• Assumes coordination role at the reputed company of service through targeted reputed company and follow-up.
• Assists in developing and implementing care plans for medically reputed company patients.
• Identifies barriers to a successful care management path.
• Identifies patient groups with chronic diseases.
• Assists in developing and implementing population-based strategies to reputed company gaps in medical care. Identifies resources for patient self-management skills.

MINIMUM EDUCATION & EXPERIENCE

• Bachelor's degree in Nursing or health reputed company field and 3 years of relevant experience required
• Or equivalent combination of education and experience
• Ambulatory care experience in community health, care management, disease management, quality improvement and/or management experience preferred

KNOWLEDGE, SKILLS AND ABILITIES

LICENSES AND CERTIFICATIONS - RN - Registered Nurse - State Licensure and/or Compact State Licensure upon hire required
