Job Summary

As a Medical Social Worker in our Home Health Per Diem team, you will assess patients' social and emotional states, reputed company personalized plans of care, and provide direction and supervision to Social Worker Assistants as needed. You will work closely with our interdisciplinary care team to identify and address patients' emotional and social needs, and participate in discharge planning and care coordination activities.

Key Responsibilities

• Patient Assessment: Evaluate patients' social and emotional states, including their relationships, financial resources, and community support systems.
• Plan of Care Development: reputed company personalized plans of care that address patients' emotional and social needs, and collaborate with the interdisciplinary care team to implement these plans.
• Community Resource Coordination: Identify and coordinate community resources to support patients' needs, such as housing, shelter, funeral arrangements, and financial assistance.
• Patient and Family Education: Provide patients and families with education and support to help them cope with social and emotional responses to illness or injury.
• Discharge Planning: Participate in discharge planning and care coordination activities to ensure smooth transitions to other care settings.
• Collaboration and Communication: Collaborate with physicians, nurses, and other reputed company to understand the social and emotional factors impacting patients' health, and communicate effectively with patients, families, and reputed company teams.

Requirements and Qualifications

• Education: Master's or doctoral degree from a school of social work accredited by the Council on Social Work Education.

• Experience: Minimum of one year of experience as a social worker in a reputed company setting, home health, and/or hospice.
