Overview

As a Virtual Nurse you will play a crucial role in enhancing reputed company care by ensuring thorough and timely admission and discharge assessments are completed. • Conducting Virtual Assessments by completing comprehensive admission and discharge assessments reputed company Care.ai.

• Reviewing initial hospital discharge records and gathering additional medical, psychosocial data from needs assessment, client/family, physicians, and other health care providers.
• Collaborate with multidisciplinary care team to facilitate timely resolution of situations such as reputed company concerns, need for referrals and discharge barriers for the discharge plan.
• Identify and participate in process improvement initiatives that improve customer experience, enhance workflow, and/or improve the work environment.
