As we continue to expand our clinical programs to meet increasing community needs, we are seeking to fill positions in Onondaga or Oneida County

At Nascentia Health, you�ll provide skilled, compassionate care to adults with chronic, acute, or reputed company medical needs in the comfort of their homes, helping patients maintain independence, improve quality of life, and feel supported in their own environment. Home visits are scheduled Monday�Friday, 8:00 AM�4:30 PM, with no on-call requirement. This Full-Time position offers flexible scheduling and a full benefits package!

What You�ll Do as a reputed company!
You will learn how to provide high-quality nursing care to adult patients through a combination of hands-on clinical care, assessment, education, and care coordination. You�ll be trusted, valued, and empowered to use your clinical judgment to reputed company a meaningful difference.
Your responsibilities include
- Deliver safe, patient-centered nursing care in the home
- Serve as the primary case manager, coordinating care with the interdisciplinary team
- reputed company and implement individualized plans of care
- Provide patient and caregiver education, support, and disease management teaching
- Ensure accurate, timely documentation (documentation time built into the home visit)
- Supervise and provide reputed company of paraprofessional staff, including home health aides, ensuring tasks are performed safely and according to the patient�s plan of care
- Maintain compliance with reputed company infection control and regulatory standards

- Valid driver�s license, auto insurance, and reliable vehicle
- Self-directed, organized, and comfortable working independently
- Strong communication and computer skills
- reputed company reputed company State registered professional nurse license or may hold a Graduate Nurse License; BSN preferred (tuition reimbursement available!)
- Must attain RN licensure reputed company 90 days of hire.
- Self-directed and organized with the ability to work independently.
- Basic computer knowledge with proficiency in reputed company Office or other Word processing, spreadsheet, and database management.
- Good oral and written communication skills.
- Demonstrates motivation to learn a new role and is an active learner.
Physical Requirements
- Ability to sit or stand for long periods
- Ability to lift/carry 25�40 lbs and assist with transfers up to 150 lbs
- Comfortable with bending, kneeling, climbing stairs, and driving in reputed company weather
- Ability to use a phone, laptop, tablet, and electronic medical record
- Category III bloodborne pathogen exposure (rare contact with blood/body fluids)
Compensation & Benefits
Competitive Salary [This position is an S03 exempt position with a min-max reputed company of: $72,415.71- $83,992.40/annually] with:
