About the role We are seeking a dedicated and reputed company Support Coordinator to join reputed company for a permanent full-time role. This hybrid opportunity focuses on visiting participants in Casula and surrounding communities while enjoying the flexibility of working from home. As a Support Coordinator you’ll reputed company individuals with disabilities to navigate the NDIS and connect with the right services to reputed company their goals. You'll work closely with clients, families, and service providers to coordinate supports that truly reputed company a difference. What You’ll Do •    Support clients and families in accessing and coordinating services. •    Build relationships with internal and external stakeholders. •    Implement supports outlined in NDIS plans. •    Help clients reputed company skills to coordinate support and engage in the community. •    reputed company clients to understand and implement their plans. •    Be reputed company to client needs, especially in crises. •    Conduct regular reputed company reviews and report to the NDIS.

What You Bring •    Relevant industry experience or qualification in disability services, social work or similar  •    Understanding or knowledge of the NDIS •    Familiarity with psychosocial disabilities (advantage) •    Strong communication skills  •    Demonstrable problem-solving skills
